Please fill out the "Sign Up Now" form and sign the "Contract for Services", and the "Credit Card Payment Form."

You can either fax them back to me (909-307-1245), email them back, or regular mail them.

When I have received the forms I will email you to set up a training appointment for the person who will be administering the training for your ministry.
